Dr. Mead

Ronald Mead, O.D.

Undergraduate: Central Michigan University, Ferris State University

Doctor of Optometry: Ferris State College of Optometry

Married to Lori, childen: Jason, Kristin, Cameron and Trevor.

Dr. Mead’s professional associations include the Michigan Optometric Association and the American Optometric Association. He serves as a Vision Source Adminstrator for Northern Michigan and the Upper Peninsula. Hobbies include: Golf, computer.


Dr. PeterPeter Oppermann, O.D.

Grayling High School graduate with honors.

Bachelors of Vision Science: 1995 Ferris State University with honors.

Doctorate 1998 Michigan College of Optometry with honors.

Dr. Oppermann specializes in the eye health examination of patients of all ages, including the management of complicated eye disease as well as recommendation of all forms of eye wear.

He married and the father of two children, enjoys downhill skiing, hunting, boating, fishing and various other outdoor activities.

Dr. Oppermann is an active member of the Rotary Club of Gaylord, the Michigan Optometric Association and the American Optometric Association. He also serves as an Administrator for Vision Source of Northern Michigan and the Upper Peninsula.


Nathan R. Slezak, O.D.

Bachelor of Science , 2006 Central Michigan University.

Doctorate of Optometry 2010 with Highest Distinction, Michigan College of Optometry

Dr. Slezak specializes in eye health examinations for patients of all ages, including complicated eye diseases. He also recommends all forms of eye wear.

Dr. Slezak is the JV Basketball coach for the Gaylord St. Mary Snowbirds. His professional associations include the Michigan Optometric Association and the American Optometric Association.


  Johanna Mauk, O.D.Johanna Mauk

Undergraduate: Northwestern Michigan College, Bachelor of Science: Ferris State University.

Doctorate of Optometry: Michigan College of Optomety

Dr. Mauk has experience in primary eye care, as well as ocular disease and contact lenses.  She also has pediatric and low vision experience.

Married to Jeff, Dr. Mauk enjoys spending time with family and her Boston Terriers, Iris and Max, trying new recipes and riding both her road and mountain bike.


Robert Slezak, O.D.Dr. Robert

Bachelor of Science in Education, 1975 Central Michigan University.

Graduate work in Biology 1976-1980.

Doctorate 1985 (FSU) Michigan College of Optometry – Valedictorian of his graduating class.

Dr. Slezak specializes in eye health examinations for patients of all ages, including complicated eye diseases. He also recommends all forms of eye wear.

He is married to Mary, has two sons, Ryan and Nathan and a growing number of grandchildren. He is an avid fly fisherman and golfer. He taught high school for 6 years before becoming an optometrist.

Dr. Slezak is a member of the Kiwanis Club of Gaylord. His professional associations include the Michigan Optometric Association and the American Optometric Association. He serves as an Administrator for Vision Source of Northern Michigan and the Upper Peninsula.


MeLissa Olson, CPOAMeLissa

MeLissa began working for Gaylord Eye Care Center, Inc. in 1991 and is the  Manager for Gaylord Eye Care Center, Bellaire Eye Center and Eyes on Main, handling administrative duties for all three offices. She is a Certified Paraoptometric Assistant. MeLissa serves as the Vision Source Representative Facilitator for Northern Michigan and the Upper Penninsula.

MeLissa is married with 3 grown children and enjoys any time she can spend with them and her grandchildren. She spends her spare time as a volunteer with the American Cancer Society as a Road to Recovery driver, she is a past mentor with the Lunch Buddy program. She spends a lot of her time volunteering for Relay For Life of Otsego County. She is also a former member of the Great Lakes Division training team and has served as the State Lead Ambassador for the American Cancer Society Action Network (ACS CAN). She also enjoys spending time with family, reading, scrapbooking and genealogy.

Office Manager for Gaylord Eye Care Center

Kami began working for Bellaire Eye Center / Gaylord Eye Care Center / Eyes on Main in 1991. She is a Certified Paraoptometric Assistant and the Office Manager for Gaylord Eye Care Center.

Kami is a “Jill” of all trades, assisting where needed.

Kami has extensive training on all levels of preliminary testing, including slit lamp evaluation of contact lenses, topography, optical coherence tomography (OCT), visual evoked potential (VEP) and retinal photography. She also does intra-ocular pressures, corneal measurements, visual fields and visual acuities.

She is married with three children. She enjoys reading and bowling.


Theresa-HaskeTheresa Haske

As our “Director of First Impressions” Theresa’s primary job is greet our patients and guests as they arrive at Gaylord Eye Care Center!  Something that she does with her great smile and warm personality.  Theresa joined our staff in 2013 and has been a wonderful asset!

Theresa also specializes in checking insurance eligibility, maintaining records, scheduling appointments and referrals.

Married to Steven, mother to Sara Lyn and Zachariah and her amazing son-in-law, Rob and one very precious and adorable grandson, Brock!

In her spare time she is actively involved in Operation Christmas Child, she also loves to spend time in her garden, and takes pleasure with her kitties and spending time in the jeep!


 TammyTammy Warren

Tammy began working for Gaylord Eye Care Center in 1999. Her duties include the our phone and call center, front desk and reception area, as well as technician. She greets patients as they arrive, assists in preparing records, answering the telephone and checking insurances. She also schedules appointments and handles referral scheduling. Tammy’s duties also include monthly statements and account receivables.

Tammy is married with two children and grandchildren. She enjoys spending time with family and friends, bowling, knitting and cross-stitch.  Tammy is also our resident baker.  She is responsible for most of our birthday treats.


 Teresa Abbott, ABOCTeresa-A

Teresa has worked for Gaylord Eye Care Center since 1993.  She became a board certified optician in 2000.  Teresa works in helping our patients to select their personal eyewear and instructs new contact lens wears on how to insert and remove their contact lenses, as well as how to properly care for them.

Teresa’s other duties include dispensing glasses, adjusting and repairing eyewear.

When not working Teresa enjoys spending time with her husband Chris, large family and traveling.  She is Nana to many beautiful grandchildren!


SandySandy Mammoser, ABOC

Sandy has worked at Gaylord Eye Care Center since 2007. She has been an American Board of Opticians Certified Optician since 1996. Her duties include assisting in frame selections, adjustments of eyewear, repairs and ordering of all frames, lenses and contacts. She also dispenses glasses, lenses and contact lenses. Prior to moving to Gaylord she worked as an Optician at an optometric office in the state of Virginia.

Sandy is married to Tom and together they have one daughter. She enjoys gardening and singing.


LizLiz Colton

Liz began working at Gaylord Eye Care Center in 1998. After many years working in the lens surfacing area she now works in both the finishing of lenses and helping patients to select the best eyewear for their visual needs.

She dispenses frames and lenses, as well as does adjustments and repairs. Her duties also include ordering frames, lenses and contact lenses, as well as being able to surface lenses.

She is mom to Matthew, but Liz is affectionately known as our resident cat lady! She does love her kitties!


JohnJohn Puroll

John has worked for Gaylord Eye Care Center since 1995 as our surfacing lab manager, but has many more years of experience in surfacing and edging lenses. Because of this, John has a vast knowledge of lenses and optics. He handles the process of surfacing the power of lenses, as well as edging and repairs. John has been known to take a “beyond repair” frame and make it wearable until the new frame and lenses are ready for a patient.

John is married with two daughters. He enjoys playing basketball, golfing and hunting.


MarjMarj Webster

While Marj joined the Gaylord Eye Care Center staff in 2011, she comes to us with 40+ years of experience in ophthalmology/optometry. Marj is an optician and has a background in ophthalmic assisting. Her  responsibilities can range from assisting in the eyewear selection, contact lens training for the new wearer, as well as ordering and dispensing eyewear. Most days, however Marj is on the phone managing our ever hectic schedule.

After  25 year of residing in Florida, Marj relocated to northern Michigan and is active in remodeling, including woodworking. She enjoys being out-of-doors. We do try to keep her off the roof as much as possible!


Amanda Kowalski, ABOC

Amanda has worked at Gaylord Eye Care Center since 2006. Amanda began working as an optometric assistant and can still be found assisting with patient testing.

In early 2013 Amanda completed Advanced Dispensing courses through Essilor’s ECP University. Her primary job responsibilities are now related to fitting and dispensing eyewear.  She now handles new lens technology training for the staff. She also handles repairs and adjustments. In November 2015 Amanda sat for and passed the very tough ABO exam and is now a certified optician.  She has also been recognized for her innovative design of our Transitions Lenses Activation Station.

In her spare time she likes to garden and play with her very rambunctious dogs.


Amy AtkinsAmy Atkins

Amy joined our staff as an optician in 2014.  She has a great personality and smile, as well as the skills necessary to help our patients find the right eyewear for every need.  Amy  can also help instruct on the proper way to insert and remove contact lenses, as well as the care for contact lenses.

Amy is married with two sons, she enjoys boating, fishing, camping  and keeping busy with her family and kitty cats!


Lauren Evans, CPO

Lauren began working with our office in 2015. She graduated from Spring Arbor University and will soon be off to pursue her degree in Optometry.  In the meantime she is honing her skills with patient care.  Previously she has experience with optical in Jackson, as well as working in a winery and as an au Pair. Since working for Gaylord Eye Care Center, Lauren has passed her Certified Paraoptometric Examination.

Lauren as also done mission work that has taken her to places like Nicaragua, Jamaica, Ireland and Scotland.  We are pleased to have LO as part of our staff, for however long we might have her and her passion for people is evident.

Ashley joined our team in 2015 first as part of our front desk staff and then finding her groove as part of the pretesting team.

Ashley took some time off but is now back to work at Gaylord Eye Care Center where you can see her beautiful smile and friendly face either behind the front desk or assisting in patient care.  She and her husband have a bouncy blue-eyed boy named Harrison.

Lisa Burns

Lisa joined our team from the office of Dr. Frank Rosenbaum where she was their primary biller.  She has a varied optical background that includes working in the optical, front desk, billing and pretesting.  Currently she is transitioning into a role as co-biller.

Lisa is married with four children.  She sees a lot of wild life on her daily commute to work and loves her cup of coffee!


Casey StinsonCasey Stinson

We proudly stole Casey from Helen’s Hallmark because of her bubbly personality and mad skills! Casey joined our staff in 2016 to begin her career as a paraoptometric. Her customer service skills and smile make for a winning combination.

Casey is a GHS graduate and has one child. In her spare time she likes to read and bake.


Cassie Taylor Cassie Taylor

Cassie joined our team from the office of Dr. Peter Fedor.  She is working as an optician, but also has the training to do special testing. Cassie completed training at Ferris State University for Opticians. Cassie has an amazing passion for learning and always willing to do more!  Her smile and cheerful attitude make working with her a bonus.

Cassie is a graduate of GHS and the University of Phoenix.  She and her husband have two children, Thomas and Madelyn.


 Mariah McLeod

Mariah joined the Gaylord Eye Care Center team with an extensive background in the ophthalmology. She also has worked as a nursing assistant. She is also an EMT.  We are pleased to have her with us.  She specializes in patient care prior to seeing the doctor and has training in special testing.

Mariah is a hometown girl.  Born and raised in Gaylord.  In her relaxation time Mariah loves to camp, enjoys fishing and helping out with community, especially Wings over Northern Michigan Airshow every year.

She and her husband have one little kiddo and a big dog!
